Frequently Asked Questions

    • Valid Drivers License

    • A list of current medications you are taking

    • Glasses and hearing aids, if you wear them for driving

    • You will be asked to provide a urine sample during your exam

  • Make sure you are taking your medication leading up to your exam, avoid any caffeine or nicotine a few hours before your exam as this can increase your blood pressure. Your blood pressure must be under 140/90 for the DOT exam.

    • please bring your most recent (within the past 4-6 months) HbA1C results. 

    If you are a diabetic who is on insulin, you will need a DOT Insulin Form MCSA-5870 filled out by your doctor.

  • Please bring a report showing your compliance using your CPAP machine for the past 90 days.

    Many people have access to this on an app on their phone, you can show this to your medical examiner during your exam. 


    If you do not have this on your phone, please call your doctor’s office to request this information prior to your exam. We will need to see that you are using your CPAP at least 4 hours per night, with >70% adherence (around 21 nights a month).

  • Please bring your most recent INR results with you to your exam.

  • We will need a letter from your physicians regarding the safety of driving while taking these medicines. The letter should mention that the medication is well-tolerated with no daytime sleepiness and the doctor has no concern with you driving with your DOT/CDL.

  • We will need a letter from your neurologist detailing your treatment, history, and current neurological state. Your neurologist will have to provide a letter stating they have no concern with you driving with your DOT/CDL.

  • We will need a letter from your cardiologist stating that it is safe for you to drive a vehicle with you DOT/CDL. 

  • We will need a statement from your physician about any work restrictions. You may also need a Skilled Performance Evaluation, in order to acquire the DOT card.
